W-4 Instructions for Those Who Don't Use Sinai Central (written)

IF YOU ARE NOT A SINAI CENTRAL USER, if you don't use Sinai Central for purchase orders, travel requests, HRTS, online grant applications, etc... Then follow these directions for filling out the online W-4 forms.

For Internet Explorer Users: If you experience an error message while using the online tax forms...

If you use Internet Explorer, you may experience an error message while filling out the online tax forms. The message looks like this:

You must respond to this error message before continuing with the online forms. First, click where it says Click here for options...:

Then, select Allow Blocked Content....

Internet Explorer will display a security warning:


Select Yes, and continue with the online forms.

How to Fill Out the Online Tax Forms

  1. Launch your browser and go to http://sinaicentral.mssm.edu/w4.
  2. At the prompt, enter your Social Security Number, Date of Birth, and Life Number.
    1. For your Soc Sec No., use the format XXX-XX-XXXX.
    2. For your Date of Birth, use the format MM/DD/YYYY.
    3. Enter your Life Number wihtout dashes or spaces.
  3. Click Login.
  4. Select Fill  Out Tax Forms.
  5. The program gives you important information about filling out the online forms. Please note that if you close out your browser before receiving a message that you have officially submitted your tax forms, your changes will not be saved. After reading the information, click the Continue button.
  6. Confirm your name and social security number.
    1. If your name or social security number is incorrect, select Incorrect. You will be given a number to call Payroll Services. Only Payroll can correct this information. Stop the process here.
    2. If your name and social security number are both correct, select Correct and proceed to the next step.
  7. Confirm or correct your address and phone number.
    1. If any of the information is incorrect, select Incorrect. You will be given a screen where you can enter the correct information. You’ll know you are in this screen because the text has changed color, and is now blue. Correct the information, then click Save, and then click Correct.
    2. If all of the information is correct, select Correct and proceed to the next step.
  8. Fill out the online W-4 form.
    1. Your name, address, and social security number are taken from the previous screens. If any of them are incorrect, you must click on Step 1 or 2 at the top of the window to go back and correct that information.
    2. In Box 3 of the form, click to select Single, Married, or Married, but withhold at higher Single rate.
    3. In Box 4, click the check box only if your last name differs from that shown on your most recent Social Security Card.
    4. In Boxes 5, 6, and 7, enter the proper information for your tax situation. If you want to use the Deductions and Adjustments Worksheet, you can scroll down to page 2 of the form.
    5. After filling out the form, scroll to the bottom of page 1, where you will see the Submit button.
    6. Click Submit.
    7. You are presented with a chance to review the form. Please do so. If the form is correct, click the Correct button. If you want to correct any information, click Incorrect and you will be taken back to the editing window for the form.
  9. After submitting the W-4 form, the system advances to the New York tax form IT-2104. If you chose Exempt on the W-4 form, you will be taken to form IT-2104-E instead.
  10. Fill out the IT-2104, just as you did the W-4. Note that even if you are not a New York City or State resident, you must still fill out this form.
  11. Submit the IT-2104 form. Again, you are given the chance to review and correct it before clicking the Correct button.
  12. If you live outside of New York City, you will be presented with a third form, the New York City Non-resident Tax Form (IT2104.1). Fill out Parts 1, 2, and 3 of this form and submit it.

    If you live inside New York City, the system will skip the IT2104.1 form.
  13. After submitting the last form, the system displays a message telling you which forms you have officially submitted. You’re done!
  14. To keep your information secure, close your browser window.
