Completing the 2009 Report of Relationships of Outside Entities

Everyone who has a Mount Sinai life number, a social security number, and a date of birth can log onto Sinai Central. Here is how you can login/activate your account :

1. Launch your browser and go to http://sinaicentral.mssm.edu.
2. On the left side of the page, select the link for Sinai Central Home.
3. Login. If you need to obtain a Sinai Central account or are locked out of the system, click the link for Login or Password Help. This leads you to a self-service system that is in place to assist you.

You may need to click on the Home link in the top right hand corner of the screen to view the top-level services.

1. Click on the document icon next to your name and the 2009 Report of Relationships with Outside Entities will appear.
2. Follow the instructions on the form and answer all of the questions.
3. Click "Save" so that your disclosure form is officially captured by the system.
4. Check the I verify question and click on the Submit button.
